You will be able to get this at the end of the last chapter. before the final boss battle.
When you reach the room where everybody is connected to the STEM (you will see yourself lying in the bathtub alongside Sebastian and all the others) don't exit through the next door just yet.
Instead go to the middle of the room where you will see Ruviks brain in the glass container, the one that Sebastian smashes at the end of the main campaign. Now simply pull out your shotgun shoot it (make sure you save up 1 shotgun shell for this).
Fake closing credits will roll and you will unlock your achievement.
Afterwards the game continues from where you left off.

Near the end of chapter 4 you will come to an elevator. After you push the button to call it, enemies will swarm you and you have to defend your corner for about 30 seconds until you can enter the elevator and go up.
It's IMPORTANT that you save at least one shotgun round for the next section.
Exit the elevator and shoot Ruviks brain in the middle of the room. After a short end credit interruption you can control your character again and the achievement/trophy will pop.

Towards the end of the chapter you will arrive at the STEM and see yourself, along with your friends, hooked up to the machine. If you've completed the main game you will recognize the brain in the centre as Rubin's. Shoot this to unlock the trophy. Melee might work too (haven't tried it).

Just to add to this, make sure that you have a bullet left. It is possible to have no bullets left at all. Make sure you have a bullet after the fight with the glowing guys coming out of insane asylum like rooms.
