Prior to this episode, you need Rayen to fully trust you. That means, in Episode 1, break up the fight between the brothers and save Rayen's leg. Then, at the end of Episode 3, you need to have chosen to trust Rayen.
In the beginning of Episode 5, the first choice is obviously to choose not to kill Rayen. Then in the medical bay you need to successfully help Virgil with his panic attack. The steps for that are in the solution for that achievement. This way, Virgil will give Rayen medical attention. Then Rayen will show up during the fight with Arlen and will shoot Arlen and the achievement will pop shortly after.

You also need Rayen to FULLY Trust you.
That means, in Episode 1, Break up the fight between the bros & Save Rayens leg.
Trust Rayen At the end of episode 3, then do everything mentioned above.
Toussaint shows up instead otherwise and you need to replay the game again.. Like i had to haha
