The Final Station

This is how I unlocked this achievement.

From Starting a fresh game to this point took me 45 mins.

1) First get to the section where you pick up a gun
2) Resce the women before the 1st enemy as this is the checkpoint
3) Go down the ladder and kill 1st enemy
4) Open man hole cover and go down it (ignore the enemies or kill them up to you)
5) Go through the next door pick up the box
6) Through the the next door throw the box and the enemy and retreat down the ladder in the previous room
7) Pick up the chair and throw it at the enemy
8) Let the enemies kill you
9) Repeat steps 3 to 8 four more times
10) Achievement Unlocked :)

This achievement is easy to obtain by the end of the game as you will always have a few items to throw and way to enemies to kill. There is a great spot to grind this out is a few restarts at the second stop of ACT 1. You'll find two boxes right outside a convenience store with 4 enemies in it, so throw the boxes, then get killed to restart and repeat until you get it. Video is showing how to get City of Factories, but contains the spot I'm talking about. Hope it helps!
