This one is really easy. Just give yourself dysentery, then let yourself die of dehydration.
Take an empty jar and fill it with polluted water, which is pretty much all standing water on land and river water when you are standing on a dock. Drink the polluted water, which will give you one of a few different conditions. Repeat until you get dysentery, then just keep your other life bars high while letting hydration drain out. Sleep to die of dehydration even faster. Got it on my first try, took 2 minutes.
