How to unlock the Sickly achievement in The Flame in the Flood - Definitive Guide

To get this achievement you must contract each affliction at least once:

Weather
- Wet: stand in rain (Does not count towards achievement progress)
- Ill equipped: wear insufficient clothes

Health
- Dehydration: deplete Thirst (potentially fatal)
- Starvation: deplete Hunger (potentially fatal)
- Hypothermia: deplete Body temperature (potentially fatal)
- Exhaustion: deplete Fatigue (potentially fatal)

Hazards
- Laceration: wolf, boar or bear attack (cure with bandage or stitch kit)
- Broken Bones: boar or bear attack (cure with splint)
- Snake bite: get bitten by a snake (cure with dandelion tea)
- Ant Bites: stand on the ant holes in the ground (cure with aloe)
- Poison ivy: stand on the large reddish/orange flower beds (cure with aloe)
- Tiny cuts: walk through thorn bushes (cures with time)

Disease
- Staph infection: leave Laceration untended (cure with penicillin)
- Bacterial infection: leave Ant Bites untended (cure with penicillin)
- Sepsis: leave an Infection untended (potentially fatal, cure with penicillin)

Food
- Food poisoning: Yucca, Spoiled meat or Tainted meat (cures with time)
- Parasites: Spoiled meat, Polluted water or Moldy lump (cures with time)
- Giardia: Polluted water (cures with time)
- Listeria: Polluted water (cures with time)
- Dysentery: Polluted water (cures with time)
- Blood flukes: Spoiled fish (looted from bear caves. cure with Sumac tea)
- Drunk: Alcohol (cures with time)

Deaths
- Drowning: die by totalling the raft (Does not count towards achievement progress)
- Wolf attack: die from wolf attacks (Does not count towards achievement progress)
- Gored: die from boar attacks (Does not count towards achievement progress)
- Mauled: die from bear attacks (Does not count towards achievement progress)

You might want to update the guide to state that you must get all of the different afflictions now to unlock. I was at 90% and desperately looking for a rotten fish to get blood flukes from for a few hours, while of course getting wet an other afflictions.

I will confirm after I leave ant bites unattended (I think that's the one that's keeping me at 95%) because I remember getting the rest for sure.

The increments (90%, 95%) suggest there being 20 total afflictions required for the achievement. I have more than that listed here so some must not be required.

Thanks! That still makes 21 afflictions though, so one more needs to go. I suspect "drunk" since it's not an affliction but a self imposed state

Since there is no fishing in the game (despite finding fishing line and hooks regularly), it should be mentioned that fish can only be found by looting a bear's cave as far as I know.

Progress doesn't go up by exactly 5% every time, since my alt has 9%. I suspect that 21 afflictions is the correct number.

Polluted water cures are wrong - Listeria and Dysentery need Penicillin, Giardia needs Sumac Tea or Dandelion Tea.

Can confirm that the deaths don't matter, but the other afflictions all counted toward my increase. Finally got it when I got a bacterial infection last night. I am relatively sure that being "wet" is the one from your main list that doesn't count, due to the other achievement where you need to have stacked afflictions at once not counting being wet.

There are many afflictions in the game. Some are easier to contract than others. They do not have to be done in one playthrough. The only one that may be difficult would be Blood Flukes. You only have a chance of contracting them after eating Spoiled Fish, and the Fish only has a chance of occurring in a Bear’s cave. Here is a list and the best way to contract each of them:

  • Dehydration (deplete Thirst meter)
  • Starvation (deplete Hunger meter)
  • Hypothermia (deplete Body Temperature meter)
  • Exhaustion (deplete Fatigue meter)
  • Laceration (Wolf, Boar, or Bear attack)
  • Broken Bones (Boar or Bear attack)
  • Snake Bite (bitten by a Snake)
  • Ant Bites (walk through an ant hole on the ground)
  • Poison Ivy (walk through the flower beds on the ground)
  • Tiny Cuts (walk through thorn bushes)
  • Staph Infection (leave a Laceration untreated for a while)
  • Bacterial Infection (leave Ant Bites untreated for a while)
  • Sepsis (leave an Infection untreated for a while)
  • Food Poisoning (eat Yucca, Spoiled Meat, or Tainted Meat)

It is necessary to get sick with all types of diseases:

- Lacerations: attacks from wolves, boars and bears;
- Broken bones: attacks from boars and bears;
- Snake bite;
- Ant bite - you need to walk through an anthill (holes in the ground);
- Poison ivy - you need to walk through the orange flowers;
- Small cuts - you need to go through the thorns;

- Staphylococcal infection: do not treat lacerations;
- Bacterial infection: do not treat ant bites;
- Sepsis: do not treat infections;

-Poisoning: yucca, spoiled meat, stinking dope;
-Parasites: spoiled meat, dirty water, mold;
-Listeria: dirty water;
-Giardia: dirty water;
-Dysentery: dirty water;
-Bloodsuckers: spoiled fish;
-Drunk: drink alcohol.

Also required are ailments that appear from lack of food, water, warmth and sleep (bring them to zero).
