There's a lot you need to do in order to get this achievement, so I'm going to note every other achievement you can get on the way. There are two ways to get this, you need either the upper cistern key or the golden bow. I outline both here. You will want to have gone through most of the game before completing this. If you are playing as a soldier, you can go for the Dead Shot achievement here too. I wrote this kinda quickly, so if I missed anything, let me know in the comments so I can adjust the guide.

Most of this will have spoiler info, so beware.

You will need either the upper cistern key from Duli, or the golden bow from Temple of Diana for this. I saved as many people as I could for this, so I will list want I did. You may be able to skip one or two of the tasks and still unlock it, but if you played through most of the game, you should already have done most of this stuff.

These tasks are not all necessary, you either need the golden bow or the upper cistern key. This tells how to get both.
Here are the preliminary tasks:
1. Obtain the medicine from Desius - either by paying 1000 denarii or by stealing - and give to Lucretia to save Iulia and get the Medic achievement.
2. Read note on right side of crumbling temple BEFORE engaging dialogue with Fabia to save her, and direct the assassin there instead. This will also unlock the Trickster achievement. Collect the wooden bow, and bounty notice from him after he dies there.
3. If you want the golden bow (and the Survivor achievement), speak to Desius after obtaining the wooden bow. Follow this questline to get the golden bow (extinguish both flames in temple of Diana, grab the quiver, then equip wooden bow and look at the golden bow to swap them. Deny Desius the bow, fire an arrow at the bee hive, and escape through the palace. This will reward you with the key to Naevia's chest in the temple of Apollo (you can also get this from her house in the slums without the sin of theft, but you won't get the golden bow if you use that quick method).
4. Unlock Naevia's chest in temple of Apollo to find her treatment notes. Speak to Lucretia to obtain willow bark to cure Rufius (if you search his home in the slums and find the half-written note about Virgil, you can get the Lion Tamer achievement when you give him the willow bark).
5. Search the back area of the slums for a chest with wine. There are two chests in this area with wine you can take without stealing. Go to the large temple with the obelisk and let Ulpius jump to his death. Follow after him to enter Malleolus' home and get the High Diver achievement. Confront him about being Quinctius and ask him to withdraw from the election. After failing to convince him, save your game. Get the key from the left side of the front door and go upstairs to talk to Claudia. I've been locked out of dialogue with her, which is why I say to save before talking to her. Choose the flirting option in the beginning. She wants to get back at her husband, but not ruin him, so be careful choosing your responses. You will need to give her wine for her to give you proof of Quinctius' identity. Once you have it, you can make him withdraw from the election.

Ok, now you should be able to get Gallerius elected and he will free Duli, which will reward you with the upper cistern key and the Liberator achievement. To be clear, you will need to sin to loop back in time, then in your first talk with Gallerius, have him cure Iulia and Rufius, direct Fabia away from the crumbling shrine and assassin toward it, and tell Malleolus of the proof you have of his identity, forcing him to withdraw from election. This will make everyone he helped vote for Gallerius. If you need a tiebreaker vote, Desius will vote Gallerius for 3000 denarii.

If you enter the cistern from the lower door, and have the golden bow, you can get the Italian Plumber achievement by shooting the creeping vines and climbing to the upper cistern. If you have the upper cistern key, you can enter from that door. Either way, you will find Sentilla. Make a save before talking to her so you can get three achievements here (if you are playing as a soldier). Speak to her, free her and shoot Sentius in the face for Dead Shot and The One That Got Away. For ending #3 and the purpose of this guide, speak to her but don't free her. Instead go sin and loop back in time. When meeting Gallerius again, have him save everyone he can and meet you in the upper cistern (all of the tasks you can have him do to get elected are not necessary for this) when the ground begins to shake. Go free Sentilla, but let her kill Sentius. You won't see the others, but some of them will have made it, unlocking The Ones That Got Away.

I had a lot of trouble getting this ending. I tried everything in the above guide with no luck and even the comment from the guide for the "Dead Shot," but, I seemed to always get ending 2. I did some searching and testing and finally got it and in a much easier way. By following the below, you can skip most interactions and get the achievement in roughly 10 minutes.

Ending 3 should now unlock!

You can get both endings if you tell her you'll get Galerius to gather everyone, don't free her, save, then free her. Get the ending 2 achievement, then reload and do what you need to do to get Ending 3.

When in the upper cistern, decline to free the girl and say you will gather the others (I had to speak to her twice for this to appear).
Then kill her and run for the portal.
Tell the guy in front of you to do something for you and tell him to gather the others.
Then simply go back the captive and free her, letting the rest play out.

I tried to make this as spoiler free as possible, avoiding character names and such.
