When Fabia requests your help dealing with the armed intruder, refuse to help. She will ask if you are callous, or stupid. Reply "Just callous, good luck!"

The achievement will pop shortly afterwards.

Thanks Jaesto! Poorly worded in the achievement description itself. You have to do it with the dialogue specified in this guide. I had her die by just letting her go in there on her own and it doesn't pop.

The trophy is not glitchy, it’s just that in the dialogues you have to refuse to help with a stranger in the bathhouse, then say that yes, I don’t have a heart. Only then does it fall out.

At the first meeting, after the dialogue, Fabia runs to the temple. The temple will collapse on her.
