Here is my 100% Achievement guide for The Great Perhaps.
This is achieved right at the very beginning of the game.
After you get shown what to press to push a cart down the hill to get across, you end up going through a house.
At the other side of this house is a grave with a teddy bear on it.
Interact with this bear using the button 8 times to unlock this.
Go to 2:05 in my video to see where and how this unlocks.
