Can be done on normal difficulty. Prologue doesn't count, can be done on any other Act. Easiest (IMO) on the final acts for each story, as all you're doing in the final acts is taking out the Boss.
Got mine on the final boss at the end of the game. Time seemed to have more to do with it though as I took a couple hits so I didn't even have a clean run but I was able to drop her quickly. Thing to remember is that once they give you the cap-crosshair you can often keep shooting her even when she's not "stunned" on the side.
Best tip to avoid her ghosts and knife shower is slow steady movements, if you jerk your hand around you're more likely to fling his feet into the path of one of the projectiles. Try to stay off the upper edge of the stage since you can't see anything spawning there before it will hit you. The scarecrow she spawns seems to have a set pattern that wasn't hard to avoid. As were her pirouettes.
