Simply beat any level (I don't think the final showdown will count) without getting hit, i.e. shoot everything. Level 1 might be the easiest as the enemies are slow, the spawn pattern is the same and after 4 plays I have not had a modifier on level 1 yet. There is a heart bar in the bottom right corner of the screen, if you get hit, you will have an 'X' instead of a heart, just in case you miss the miss message and you'll have to try again. I got this on the first playthrough without knowing what the achievement was.

I think the final round did count as it unlocked for me then, hence this has a lower ratio than completing the 13 waves.

This is actually pretty easy. Just shoot everything on a level (which signifies that you didn't get hit). The Shotgun and Flamethrower work really well with wide range.

I got this super easily on the level where you get to use a flamethrower. I didn't even know this was an achievement, so I wasn't really going for it.
