This one is pretty straightforward. You should pick a play that you have played a lot and are comfortable with. For me, I picked the first play (fighting the oil baron) which I had been working on, getting gold medals for all the acts and doing each on hardcore.
Once you pick the play, start at the first act and make sure you never dip below two hearts. If you've been getting gold medals on the acts, this shouldn't be an issue. When you finish the act, hit continue and do the next. After you beat the boss of the play, the achievement will pop.
This can be done on normal mode - it does NOT have to be done on hardcore.

In a play you have hearts that represent health. A play consists of 5 acts. You must complete the play without dropping to only one heart. You can add health by shooting hearts that appear along the level. The best play to do this on is the first play.

If you do mess up on an act, you just have to replay the acts that you dropped to 1 heart on. I didn't get this on my first playthrough, but it unlocked at the end of 1-2 when replaying. (I must have dropped to 1 heart during that act the first time around.)
