How to unlock the Why Would You Even Do This? achievement in The Gunstringer - Definitive Guide

From the main menu, go to Special Features >> Bonus Store >> Exclusive Content.
If you have earned enough cash from playing the game, you can purchase game modifiers from this section. (I am not sure, but some of the content may need to be unlocked before purchasing by reaching certain criteria in-game).
The modifiers that count toward the achievement are: Sepia Mode, No-Fail Mode, Meatstringer Mode, and Ghost Stringer Mode.
Once you have purchased all four of these modifiers, go to the level select and choose an act (Please note that the Prologue does not count for this achievement).
Once you select an act, you will see a screen where you can toggle Hardcore Mode or Commentaries. There will be a box on this page that will default as "0 Modifiers". Highlight the box and you will be prompted to edit the modifiers used for your game.
Select all four of the above mentioned multipliers and start playing. Once you finish the act, the achievement will unlock. Again, I can confirm the Prologue doesn't count, and would imagine the Epilogue would not count for the achievement either.

For this achievement you must complete an act of a play with every modifier enabled. There are four modifiers. Ghoststringer mode, Meatstringer mode, No-Fail Mode, and Sepia Mode. You must first buy these modes before you can use them. To enable modifiers, select the play you want to do, select the act, and finally select modifiers. You can choose which ones you want to enable. Just enable all of them for this achievement. This can be done as early on as you want as long as you have bought those four modes from the bonus store.
