This doesn't unlock for a normal game over if you let the time run out. You must click continue and die over and over again until you are out of *credits* and get a short scene of your guy falling over.

Mine popped after using 9 continues for anybody reading this, just to give an idea of how many times you need to die and continue

This achievement requires you to use up all 10 continues and then die and have game over. You cannot just die and let time run out and have game over that way. You need to use up all 10 continues first.
It's virtually impossible you'll do this naturally, just because 10 continues is such a massive amount of retries. If, like me, you still need this achievement when you're done with everything else, turn off God Mode (reenter the cheat at the main menu to deactivate it), then start up a new game with it set to ARCADE difficulty (so you have less health and take more damage). Now just idle on the first enemy of the game and let them kill you over and over. Make sure to continue each time you run out of health.
After your 10 continues are used up and you die again, you'll get a cutscene of your character falling over and dying. This triggers the achievement to unlock.
Note: This is obviously not possible with the Infinite Continues cheat on, so make sure you deactivate that at the main menu before attempting this.
