At the very beginning of he Island of Terror, is the Dreamer. Interact with him and complete the fetch quest of 5 items. You honestly can’t miss him, as he’s stood at the start with a big green ! Mark above his head. The only way you can miss this, is if you ignore him or kill him instead. Otherwise, an easy achievement.

Hey everyone, have three achievements in this video. They all take place in the same zone, and are really all done at the same time. When you first enter the Island of Terror you will see an abomination with a quest. It will ask for 5 Rust Fragments. They're not to difficult to find, and along the way you'll find an Island with a Bolter item. When you pick it up it will give you A Vision of the Grimm Future achievement, and as you keep going towards the last parts you see a cabin you can interact with for the Cabin in the Woods achievement. After you collect all 5 Rust Fragments, return to the quest giver and you'll complete that achievement. Hope the videos helps!
