After reaching the Third Circle and facing off against the Chapter 2 boss, there will be a Portal to leave. Check the map picture below. To the right of it, is a floor portal, which you need to enter. Keep following these around until you reach a small boss fight. There’s enemies along the way too. Once you’ve finished them all off, your achievement will unlock.

This one and the I came for dessert are glitched for me. It even shows as completed in the achievements section if the menu.

I had to wait a week for an achievement to unlock for me in this game. Not sure if it’s the game or the could servers.
