How to unlock the Tough Cookie achievement in The Incredible Adventures of Van Helsing III - Definitive Guide
I achieved this on the Valley of Frost map on Fearless difficulty. I took a standard attack from a Veteran Rubezhal, and according to my statistics I took 1,023 damage from it. I would think it's fair to say 'extreme damage' is equal to 1,000 damage, which is easily survivable at that stage of the campaign.
If you don't have more than 1k hitpoints, be sure to level up your Body skill, through ability points or equipment.
Edit: If you're defences are too high try tanking The Daimon Judge Veteran in the Temple of Lost Hope. Thanks to ICE Faux Pirate for finding this alternative.
Good Luck!

This seems impossible with the Protector class. I took Katarina out of ghost mode and removed her skills that helped me, removed the Ward Wisdom aura, took off all my armor, reset my perks that added defenses, and did pretty much everything I can to lower my protection and defenses but I can't seem to get an enemy to deal more than 932 damage to me.......ugh

FINALLY. It took me 3 hours to find an enemy that was able to do it. The Daimon Judge Veteran on the Temple of Lost Hope map did 1,344 dmg to me. I think my physical and elemental resistances were somehow still too high which is why other enemies attacks (including the epic treeants and bosses from Bounties) couldn't do over 1k. This Daimon shoots poison bolts that do massive damage.

You have to survive combat damage of >1000 from a single target. You should be at a point in the game where you are able to have >=100 skill points on Body. Once you have this, the setup to go for the achievement is as folllows:
- play a single player Scenario game
- set Scenario difficulty to Fearless
- make sure character defense is 0 (by removing all items that add defense)
- add ability points into Body until HP is >1000 (preferably >1300)
- play the Thornwood map, and make sure you don't have any Scenario modifiers named 'Marked by ...', which adds 25% damage to enemy types
- take damage from the Twigbeast a few steps into the map
Hope this helps.

Doesnt work for me. Twigbeast barely does any damage even with defense of zero. Like maybe a hundred out of the thousand?

This requires that you take 1000 damage from a single blow, and survive. Best done once you've got the "You can be strong..." achievement, which means you have 100 Body and a lot of health to survive.
Once I beat the story, I respeced and dumped all my points into Body for over 1500 health. I made a Fearless Scenario map and got the wooded map with the treants. I took off all my armor and let him smack me for about 1400 damage to easily unlock the achievement.
