Make sure to go to the hotel first when arriving to Innsmouth.
Dialogue with the concierge doesn’t affect the achievement.
Pick “go to your room”
When going up to the room, you will want to make sure to go to the other wing and not directly to your room.
You will find a key on the floor, pick to look for the room that goes with the key.
Have a look around the room and discover a bit more about Dahlia Marsh. Choices in the room won’t matter.
You can then go our and head to your room.
Pick “Rest”
You will then have a nightmare, choices in this sequence don’t matter.
When you wake up, walk around the room. A portraits eye’s will be following you.
Take a closer look at it.
Prick one of the eyes with a pencil.
You will then automatically look out the window to see a mob.
Pick “take flight.”
Pick “the abandoned room”
You should then get the achievement, if not keep going until the end of the chapter :) choices won’t matter after surviving the night.

When you arrive at Innsmouth, get the hotel voucher from Muriel at the city entrance and head to the Gilman Hotel. Check in, then take a rest. Survive your nightmare by facing the thing chasing you, then barricade the door once you've woken up.
