Just after finishing memory 5 you will climb down a rope in search of Gorsky and reach The City. Take the path to the lower right side of the hole and follow it along and you will reach the first bridge up top.
You will first fall through a hole in the bridge which is unavoidable, you then want to walk around the bridge and fall through another 5-7 times and this achievement will pop.
