The mask is from The Sexy Brutale. You will find it in Swan's room on the back wall to the right. His room is a small room on the right by the main lobby stairs. You will see a picture of him on the nightstand

Another allusion to a prior TequilaWorks game, this is a mask from the game The Sexy Brutale, this is the main character's signature mask. You can find this in Swan's room, under the right set of stairs in the foyer. It will be in the back of the room, closer to where the stairs would be, hanging on the wall.

You can find the mask hanging on the wall of Swan's room, underneath the main stairs (right side) of the entrance foyer.

The achievement didn't unlock until I moved it toward my face as if to wear it, but I'm not sure if that's required or not.
