How to unlock the Roomerang: We Were All Rooting for You! achievement in The Jackbox Party Pack 9 - Definitive Guide
I was able to accomplish this in a 4 player game by making sure every player had the exact number of points going into the first elimination, then having the elimination be a tie between two players (e.g. players A and B vote for C, and players C and D vote for A).
Based on some other testing, it looks like the game uses some hidden tiebreakers when two players get the same number of votes, so I was unable to replicate it on later rounds even when the two players had the same score.

after round 1
*if there are exactly two (2) players tied for the most elimination votes
*if a double elimination has never happened in your game session
if ALL of these conditions are met, there is exactly a 50% chance that you will get a double elimination
