To receive a Breaking Card, one of your agents must suffer a Stress Break during a mission. They will then receive a Breaking Card from the Undrawn Hand. Looking at this card, you will notice the small hourglass symbol next to it. This is the time in weeks it will take for the card to expire and vanish on their own.
Before the card has expired normally, the agent with the Breaking Card needs to participate in a main mission. If the agent makes it through the mission without suffering another Stress Break, at the end of the mission, the card will be removed and the achievement is yours.

One little correction: The breaking card will only be removed (and give you ink) if the agent makes it through his next active mission without suffering another stress breakdown, otherwise the card stays and you can try again to remove it on the next mission.
To make it through a mission without suffering from a stress break, try to keep that agent more in the back and let another agent draw the enemy fire. Avoiding stress is less difficult on missions against Marteau and Strum than it is on missions against Nicastro, because Nicastro's sea monsters deal additional stress damage.
