To unlock Kogai Doors, you need to reach a certain point of the game where you watch a memory of Marie to unlock the pin. With this pin now in your inventory, return to any of the Kogai Doors you encountered along the way.
A note for the door itself, they require you to input a combination of
,

, 
, 
etc etc and it doesn’t give you much time. If you fail, you will be blown back but no damage to health is done so you are able to retry without penalty. Also, the combination stays the same so you can learn the code piece by piece.

I opened 2 and it didn't pop, then I turned auto solve on and it popped on the next one.

Is this bugged for anyone? I’ve opened 3 Kogai doors and the achievement has yet to unlock.
Edit: After turning off the option to auto-solve puzzles and exiting the game, I was able to get this to unlock after completing my next Kogai door
