Not a complete guide, but a note. If you have both eyes by the time you fight the final boss, you will get both endings for refusing the throne. I got all 3 endings by accepting the throne first.

I can confirm this still works with patch 1.5.12434.31.0. Both achievements for the endings after refusing the throne and having both of Esk's eyes still do stack, they both unlock during the ending sequence.

After you beat Lord Patryk Laddak in the Junas Ministry, a warp will open up in the center of the Oxnevylle Manor. This warp will take you to Wonder of Illygarth. This is just one small area with a save room, an empty room, and the final boss room.
Make your way to the east to the boss room. When you get there you will have a discussion with some of Eryk's old friends. After some talking you get two options; 'Accept' or 'Refuse'. Select Refuse and you will have to fight Caterina and Annabella. This fight has two phases so be ready. After you beat them the ending will begin and this achievement will unlock.
If your coming here from Esk's Secret (15G), the reason I brought you here is because in order to get All Will Be Revealed (70G), you have to have both of Esk's eyes and Refuse the Throne. I wasn't sure if you could get both achievements for refusing if you had both eyes in your inventory so I chose not to get one of the eyes until after I did this achievement. After I refused the throne and beat Caterina and Annabella and got Embrace the Cosmic Voice (70G), I went and got the second eye and came back and refused again with the eyes and it let me get All Will Be Revealed (70G). I'm not sure if I overlooked something, but i did it this way to be safe so I could get all three ending achievements. Please let me know if you did it different.
