After you beat Lord Laddak there will be a warp that opens in the middle of Oxnevylle Manor. Take this warp to the Wonder of Illygarth. Proceed right and you will meet Caterina and Annabella. The ask you to take the throne or refuse the throne. Refuse the throne with both of Esk's eyes in your possession and you will fight Medeya right after you beat Caterina and Annabelle. If you die fighting Medeya you will respawn and only have to fight Medeya again and not Caterina and Annabella. See the achievement above for info about how I did this. I made sure I didn't have both eyes at first and I refused and beat Caterina and Annabella. After that I got the second eye and then refused and did the final ending achievement.
