I would get your counters out of the way on the normal playthrough as they should be completely absent from the extreme playthrough. When enemies have a tell-tale red glow around them they are about to attack. When the attack is about to land on you that's when you want to press 
to successfully counter.

Blaze, Naruto, Shippuden
Self-explanatory, you'll get this naturally without even trying. Use 
to block an enemy's attack just as it hits to perform a powerful counter attack. If an enemy flashes red, that means they’re going to attack, so keep your eyes peeled. Some counter attacks require extra button inputs after blocking. These inputs are random but you have a brief moment to enter the commands.
It should also be noted that Clash Mode might count as a counter too. I unlocked the trophy after doing about 6 Clash Modes in a row against Chapter 4: Counter Attack's final boss battle. Oddly enough this Chapter is easily the best chapter to farm counters on. There are even two robot boss battles here where Korra relearns Earth bending again by countering. You can counter the robot until he dies which would give you about 20+ counters just from that boss fight.
Additionally, if you're struggle with counters then you can always equip the Talisman: Two Edged Blade. This item will sometimes trigger a counter attack when you take damage from an attack.
