Story related and cannot be missed.

It is located in the Ceremony level. Follow the story stuff and make your way up the giant cake, climbing the platforms and clouds. Then talk to the character on your right to unlock it.

This is story related and cannot be missed. This unlocks in Chapter 6, 'The Ceremony'. After you've aligned the four crystals in the room with Batman's grapple gun, climb the rainbow cloud obstacle course. When you get to the top you will see Mayhem on the ground. Talk to her and the achievement will unlock once you do.
