How to unlock the Faithful Cartographer achievement in The Long Dark - Definitive Guide
Edit: This guide does not contain all of the new regions. In addition, Hinterland has announced another new region. More regions = more locations, so if you're looking to do this, better sooner than later.
Note: This is a complete rewrite of this solution. Congratulations to derechtegraf1 for completing this first and proving that it does work!
That being said, who knows if new updates might affect this achievement later. I ran this on a new save.
These locations are according to the official forums, posted by the admins. You must actually map each location using charcoal for it to count, which can be annoying since a lot of charcoal can be heavy. I suggest putting a stash in each region where you can store necessary supplies.
Each location may be one of two types: Icons and Location Triggers. Every location will show up on your screen as having been found. I suggest you map every time that happens. Also, make sure that locations that you find that are icons actually show as icons.
Here are the locations by region. Also notice that as of the patch that fixed the bug, there is now a brand new region: Hushed River Valley.
For the record, I finally popped this when mapping an Ice Cave that had an icon, but nor did it not show up as a location trigger, not was marked in the list below. There were also 2 locations on the list in HRV that I did not map. Just map everything to be sure, including every time you transition
At the time I popped the achievement, I had found 211 locations and explored 89% of the world, so 100% is not necessary.

Watch out if you're trying to use vista pictures in Ash Canyon. The one to the SE, when I mapped it, instead of getting the 75%-ish map completion that other vista pictures give, it gave about a 10% increased radius with no extra landmarks mapped. Best to not incorporate vistas into your strategy for Ash Canyon. If you strategically plan around it with the assumption of it acting like other vistas you'll have a bad time.
On that note, it's almost always worthwhile to seek out vista pictures then complete them. You'll get a huge part of the map automatically completed in most cases. Hushed River Valley, especially, if you luck out and find one early you can get in and get out before the cliffs or a sudden blizzard has the chance to destroy your run. Got everything done besides the furthest SW stuff with one vista.
Between the two tough sections - Hushed River Valley, Timberwolf Mountain + Ash Canyon, do the latter first. The summit of TM will kit you out with perfect clothing for all slots (on Pilgrim or Voyager), and a ton of coffee you can microdose right before rope climbs to make them all go smoother. Make sure to bring a hacksaw to open the plane's metal crates. You'll also get enough MREs to have very weight efficient food for making Ash Canyon go smoothly. The gold mine in Ash Canyon has a backpack that doesn't take up any gear slot that gives a permanent +11lbs carrying capacity, and crampons, which makes rope climbing take less stamina. Crampons can't be repaired, so just put them on for climbing and then take them off so they last forever. Having both with you makes HRV a lot easier. HRV has the moose hide satchel at the signal fire for another +11lbs carrying capacity, but it takes up an accessory slot, and is harder to get to, and spawns in 1 of 2 random locations, so you can't beeline straight for it like the gold mine.

Sashamorning has already made a wonderful guide that helped me get the achievement and you should refer to her guide as a reference for the other areas.
Now with an Navigation update and with a new area Bleak Inlet added to to game I figured I’d add my own guide covering the new content and tips for making this easier and faster to get.
For example now there are Polaroid pictures added to the game with 1 being in every area. If you find one and got to map the area that it tells you to go too you’ll survey a large portion of the map and can get many places marked at once, so keep your eyes peeled!
Bleak Inlet has 2 access points; through a cave system in at the bottom of the Forlorn Muskeg area, and the other is found by repelling the a cliff in the Ravine area.
You can get to the Ravine by following the train tracks in Mystery Lake out by the Dam, or by following the train tracks out from Coastal Highway. The climbing spot is near the fallen tree bridge. If the rope doesn’t spawn already attached search the area in Ravine for the guaranteed rope drop.
YOU SHOULD ABSOLUTELY GO THROUGH THE RAVINE ENTRANCE YOUR FIRST TIME
You’ll come out on the highest point in Bleak Inlet near the Radio Tower and the Pensive Lookout tower, you need to get the cannery Workshop code from the radio tower and you need to attach the climbing rope near to Lookout tower (the rope is hung on a hook by the lookout door). You CAN NOT reach this area if you take the Muskeg entrance.
Here are the names of the 17 locations you need to map in Bleak Inlet. And make sure to use the new Surveyed Locations feature in the Collections section of your Journal to see what areas you have and haven’t marked.

The only problem areas I had trouble getting unlocked in my tracker was the *Workshop in the cannery area that you have the code from the Radio Tower (the code is 154, but that doesn’t matter since you need to grab and have the note in your inventory) and wait for an Aurora, as well as complete a neat climbing and platform puzzle to get to the workshop.
The Cannery itself wouldn’t register either until I tried mapping the area from the top of the hill on the highway nearby.
The *Frozen Delta was the most annoying. It’s on the Left part of the map and it’s not just one thing but the entire frozen river with all of its branching paths, once all of that is mapped it should trigger as discovered.
Now I did this on my 500 days Pilgrim save with the only feat I had unlocked at the time Free Runner equipped.
However you can unlock this and other achievements in the Custom Mode, and I recommend using that mode for this.
Note: You aren’t able to make any Feat progression while in custom mode, so if this is your first play through you should play on Pilgrim instead so you can unlock those as you play and have them for later play throughs. But If you’ve already done all that and just want to get this as fast as possible then Custom Mode’s your friend!
You can make the animals non-aggressive, increase the loot spawn chances for everything, disable blizzards, and most importantly you can disable to Sprain ankle/wrist system so you don’t have to worry about it whenever you go up or down steep slopes. You can also make the days and nights longer but I kept it at default since I’m used to that.
The customization menu is a little confusing to use so read carefully and if you think you made a mistake just make a new save.

Great info! Since you talk about custom modes, I would suggest mentioning to NOT disable the aurora, or else you can't combine this with the skill achievement, if that's something they're going for.

Agreed. The wildlife won’t attack if you disable their hostility, even during an Aurora so you might as well keep it on.

This is a daunting one. It's best to do this late game when you know where everything is or as you're exploring the Island.

The best way to do this is to survey the area as soon as the location appears in the top left corner of your screen. You will also have to mark the icons on your map, such as map transitions, mines, maintenance sheds etc.
