How to unlock the Losing a Child is Like... achievement in The Long Dark - Definitive Guide

This achievement is missable and at the end of the first episode of Wintermute. When the grey mother gives you the key to the chest in the upstairs bedroom, she also gives you some pearls. Make sure that before you use the climbing rope to leave town, you head north along the main road. (Take a right when walking out the front door of the grey mother's house.) Once you use the rope it breaks and you can't get back up.

Following the road you'll reach a church on your left. Across the road from the church is a graveyard. Find the gravestone
and place the pearls on it. The achievement unlocked for me here, but the side quest isn't quite finished yet, so if it doesn't pop for you here try going back and speaking to the grey mother to finish the quest line.

Story related can't be missed.

You get a mission from the Grey mother for which you got to go to the Milton Farmhouse, in which you'll find a criminal, you can save or kill, so far the choice doesn't impact the story but impacts how your character response to other people, feel free to make your own choice. Inside the house find the key, with the key go to the bank, inside the bank open the safety deposit box and return to Grey Mother with the box content. Now go to the graveyard at the church and return to Grey Mother. This will complete the quest and reward you with climbing gear and a way out of Episode 1.
