In Chapter 7 after being caught by the elves and returned to Thranduil's Halls they warn you not to touch the fish.
To unlock this achievement simply dive into the lake and catch a fish. I am unsure if you need a free food slot but you can easily jump off a ledge to damage yourself and then eat to free one up.
You can also use Gollum vision (LB) which highlights the fish in blue.

After progressing through River Path and getting recaptured by the elves, you'll need to follow them after regaining control. Shortly after you start following them, you'll come to a bridge with a big pool of water on your left. Jump in the water, go underwater, and locate a fish. Swim up to it to get a prompt to catch it. Do so to unlock this achievement.
