For this achievement you have to Ring all the bells at the bell tower. This is in Chapter Eight: Her Eyes
There are 6 bells in total. 5 of them are on the way to the tower with a final 6th one found after you have climbed up the tower looking for food.
All you need to do to unlock the achievement is jump into them to ring them. They also appear glowing white in Gollum vision.

Relatively early in the chapter, you and Mell will reach the bell tower area, and the camera will pan to show you the area. In this area are five big bells, and to unlock this achievement, you just need to either throw a rock at each one or, preferably, just jump into it to make it ring.
On the approach up to the bell tower, you can find four of them. They're big, glowing white, and right on the path, making them very hard to miss. The fourth one is off to the right just before the tower, in case you didn't notice it over there. The fifth and final one isn't accessible until you enter the tower with Mell, and then get the objective to find food for her. You'll climb up the inside of the tower and then exit near the top. Follow the objective markers to track down the food items, and you'll unavoidably walk right past the fifth and final bell to ring.
