The Dragon Spear (Shaz'akhnaman) is crafted at the Great Mithril Forge, located in the Redhorn Lode (or Angry Caverns). It is a Masterwork item, so it will replace whatever Masterwork you are currently using!
After entering the Angry Caverns for the first time, following along the path should land you in front of the entrance to the Mithril Forge. Fight off the enemies, activate the teleporter, and move to the statue in front of the closed doors. This is another statue you have to sing to for over a minute in order to get the doors open. Once inside, like always, rebuild the forge by placing the missing pieces into their designated spots.
The Dragon Spear can be crafted with 5 Mithril Ingots, 9 Diamonds, 6 Ironwood, and 6 Scales.
Mithril can be found in the Darkest Deeps - A location you can enter through various areas in the map with crevices in the middle of the room. If you don't know what I'm talking about, look for a gash in the ground in a room that's a dead-end. You build down into the dark crevice and end up in the Darkest Deeps, where spiders and other monsters are, and can find Mithril ore and other precious stones (sun-stone for example). Diamonds can be found throughout your playthrough in various chests and drops. There's also a chance to mine them in the Deep Dark mines. Ironwood might be a new wood for you, but it can be found by breaking down wooden objects/buildings/structures in and beyond Dwarrowdelf. Scales are a drop from Wyverns.

Yes Hurp, I was incorrect - I thought I got my mithril in the Mithril Lode after finding the forge. You will find mithril in the darkest deeps, the crevices found in a handful of rooms that require you to build downward into them.
