In order to craft Durin's Axe, you must first find all 5 fragments scattered throughout the map.
While the playthroughs and maps are slightly different for everyone, there is at least the certainty that there is a fragment to be found BEFORE you enter the Elven Quarter, and there is a fragment located inside the area where you fight Bolgakh.
Others can comment on where they found fragments to see if it will help, but hopefully you will come across them throughout your playthrough and will have no problem crafting the Axe when it comes time to progress the story.

The crafting recipe calls for: 3 Durinul Iron Ingots, 3 Sun-stone, 4 Ironwood, 1 Fine Leather
Durinul Iron Ingots are crafted using a Salt Bath inside of a base, and Durin's Axe can only be crafted using Durin's Forge (same with the Durin set of armor).

I got the same first two pieces you described. The 3rd one was within a troll cave on the lower deeps. There is a green troll that is stronger than normal and he comes with a regular troll and a dozen goblins.
After you beat them all. The 3rd axe piece is next to some piles of treasure.

I got the 4th piece mission when I opened Balins Chest (outpost chest you gotta forge the key for) next to the chamber of mazarbal stonemap.
