How to unlock the Relentless achievement in The Lord of the Rings War in the North - Definitive Guide
I got this playing as the female elf character Andriel. I was playing in Chapter 2 Part 1 where you fight the Barrow Wight in the cemetery. I found the tombs was a better spot as there was no archers or ranged attackers to break your streak. The tombs are at the end of the first level you leave the ranger camp. You will know you are in the tombs because a side quest will trigger to clear the tombs.
Note: Other locations that work well are the challenge maps and in Mirkwood, the forest with all the spiders.
A hit streak did not start for me until I got a critical hit. To get a critical hit use your regular melee (X button) until the enemy has a yellow triangle above their head, then use the heavy melee (Y button). Once I had the streak started I backed away and used my ranged attack (LT and RT). You get about 5 attacks in a row and then need to wait for the ranged attack to regenerate (takes only a few seconds). Keep an eye on your magic meter (blue bar under your health) and use a power potion if you run low (left on the D-pad). Keep your distance so you don't take any damage (if you take damage you will reset the streak). If you get surrounded by enemies you can roll away to avoid taking damage (B button). The timer on keeping the streak going is a little quick so don't get too far away and make sure there is another enemy to continue to attack in case you kill the first enemy. If you are using the dwarf or ranger keep an eye on how many arrows you have as you can run out.
I was playing on normal difficulty when I did this. Any questions/changes I should make, comment below please. Good luck!

Edit 2: It has been noted that ranged attack for the dwarf and ranger do not add to the counter. Only the elf has this ability. Credit to Sindarin Eagle.

Note that, so far as I've been able to tell, ranged attacks only add to the hit string for Andriel, not for the human ranger. If you're playing the ranger, you'll need to do this in melee. Dual-wield, as noted, helps immensely.

I unlocked this one just playing through the story on Hero mode. I played using the Ranger class, dual wielding, and unlocked it during the chapter 3 Boss - Bargrisar the Stone Giant.
The aim in this fight is to hack his legs to pieces, and then when he falls repeat and slice up his arms until they too go out. Once his arms go out, you wallop on his head and that's the end of this boss. If you are able to keep hitting him, you'll eventually get a little yellow triangle to pop up above his head. Hit 'Y' (your strong attack) to land a critical hit, which will enter you into Hero mode. Once in hero mode, each time you strike him with either of the weapons in your left or right hand, you will rack up +1 to your combo. Dual wielding pressing the 'X' button (quick strike) will make this quite easy to achieve. If you can get two friends to join you, it will prove to be even easier as they can distract him from smashing you to bits in the process. I think I ended up with ~68X combo with barely any effort.
