The only way to die permanently in this game is at the end of a run after you have called the head magister. You have to die to the one you accuse. When you accuse someone, as long as you have a high enough percentage they will attack you. Anything under 90% ended up having the magister finish the investigation and the accused didn't fight me. I accused the wrong person but was at 90% and it still worked.

-Don't bring a companion or a pet into the final fight to make it easier to die.

You receive this achievement for dying at the end of a run. The easiest way to get this is:
1) Go immediately to the inn. Talk with the innkeeper there, getting the information about the barmaid. Also perform a perfunctory investigation while you're there.
2) Go back to the docks, and pay the barmaid to calm down, and give you the information about the messenger at the signal tower. Ask her again about the messenger.
3) Go further into the docks, and talk with the sailor (the messenger). Get the information about the signal tower being down.
4) Go to the signal tower to investigate! Which is to say, talk with the person in charge, exhaust his options...and then go ahead and tell him you're ready to make your final accusation as well!
5) Go back to the docks one last time. Accuse anyone, and have your boss (accurately) call you incompetent.
6) Take umbrage to your boss's complaints, and challenge him to a fight to the death!
7) Have your run end; tragically for your character, triumphantly as you grab a new achievement!
