An important note is that The Medium does not have chapter select, or achievement tracking, and all collectibles must be collected in the same playthrough for the achievement to unlock. However, some collectibles can be tracked through an in-game menu.

If you're following this video (which is otherwise great), be aware that it misses the very last "Troubled Man's Note".
In the same room as the Business Card -- Thomas's workspace in the final bunker -- there is a desk with a spider's web of diagrams above it. Inspect those diagrams, then look down at the table for the 20th & final "Hypothesis" note.
Otherwise, it's a great guide. Just be aware you'll need that extra right at the end.

Be careful with A New Companion. It’s the only one I missed because the game put me into a cutscene when approaching it, then started me right in front of Regrets after the cutscene. After picking up Regrets and interacting with the other items in the tent, you automatically leave the area and can’t go back.

There are 11 Postcards that you'll need to find for this trophy. While description calls them "Frank's postcards", they will be labeled as "Groundskeepers Postcards" in the game. Their locations are as follows:
Postcard #1 - "Niwa Says Hello"
When you first arrive at the gate leading to Niwa, you won't be able to go through and will need to find another way past it. Before you do, head into the small building just to the left of the gate and on the ground will be a suitcase. The Postcard is in the suitcase.
Postcard #2 - "Honest Work"
When you reach the parking lot of the Niwa Resort, make your way to the left side. On the resort side of the parking lot will be a lone car. The Postcard is on the seat inside the car.
Postcard #3 - "Promotion"
When you get inside the Niwa Resort, across from the reception desk will be a small area with a stool in it. The Postcard is on the stool.
Postcard #4 - "Someday"
When you have to look for the Bolt Cutters to get into the Dayroom, you'll eventually find yourself in a large pool room. Go through the doorway at the head of the pool and through the next hole in the wall. On the right side of this locker room is the Postcard sitting in a box.
Postcard #5 - "A Scenic View"
After you've progressed through the large Pool area, you'll eventually reach the Examination Room and will shortly after find the key to the door here. When you've unlocked the door, you'll be on a round balcony. Go left (Marianne's right) and you'll come to a dead end. On the ground on the right is a trashcan. The Postcard is inside the trashcan.
Postcard #6 - "Won't Stop"
After the first section where you control Thomas, you'll be back to controlling Marianne. When you regain control of her, you'll have to make your way through a forest. At the start of it (after you go down the initial steps), you'll come to an intersection in the path that is split 4 ways (left, up, right, and down/back the way you came). Take the path left, then the immediate left just after. The Postcard will be sitting on the bench on the right (Marianne's left). This will be just next to an Echo that you need to collect.

Not too long after the previous Postcard, you'll go down some stairs and reach a viewpoint (there will be a large sign on the left and a telescope just ahead). To the right of the telescope is a bench with this Postcard on it.
Postcard #8 - "Harder and Harder"
Just after the previous Postcard, you'll find yourself on a trail (Educational Trail). Shortly into it, you'll reach a split. The left path is the "Landscape Trail" and the right path is the "Rare Tree Exhibit". Take the left path (Landscape Trail) and a few feet in will be a bench on the left. The postcard is on the bench.

When you first meet the dog, he'll want you to follow him up a hill. Ignore him (just for now) and take the path to the right of the hill instead. Hug the left side and you'll eventually be able to interact/pick up the Postcard.
