How to unlock the Devouring Darkness achievement in The Medium - Definitive Guide

The Medium contains at least 79 collectibles required for achievements sorted into various types. The important ones include:
- 32 Echoes (Some echoes are also Henry's Echoes)
- 10 Memory Shards
- 11 Postcards
- 19 Troubled Man's Notes (Thomas' Notes)
- 7 Mentor's Diaries (Richard's)

An important note is that The Medium does not have chapter select, or achievement tracking, and all collectibles must be collected in the same playthrough for the achievement to unlock. However, some collectibles can be tracked through an in-game menu.

You can also find collectibles such as photos, eerie drawings, and "other", but these are not required for the achievement. You can see all the collectibles in this video below:

If you're following this video (which is otherwise great), be aware that it misses the very last "Troubled Man's Note".

In the same room as the Business Card -- Thomas's workspace in the final bunker -- there is a desk with a spider's web of diagrams above it. Inspect those diagrams, then look down at the table for the 20th & final "Hypothesis" note.

Otherwise, it's a great guide. Just be aware you'll need that extra right at the end.

There are a total of 7 pages to find from Richard's diary but they're listed under "Mentor's Diaries." The first one can be found in Ursula's bathroom after jumping down the hole. The next five are found relatively close to each other in the Dayroom and the last page can be found after going down the stairs through the moth formation. Please follow the video below for their exact locations.

Missable. 

There are 7 Diary Pages that you need to find in the game. While the trophy description calls them "pages from Richard's diary", they will be labeled as "Mentor's Diaries" in the game itself. These will all be found within the Niwa Resort (this will actually be the first full collectible set trophy you will earn). None of these are story-related, so you'll need to definitely keep an eye out for them. Their locations are as follows:

Diary Page #1 - "An Apology"

After you activate the first Spirit-Well with the Dried-up Sunflower, head towards the exit door, but don't go through it yet. Instead, go left and you'll eventually reach a Bathroom. In the Bathroom sink is the Diary Page. This will be right next to a Medicine Bottle that has an Echo in it for Calling Out to Me trophy_gold.png.




Diary Page #2 - "A Very Special Boy"

When you've gained access to the Dayroom, there will be a couch on the left side of the main room. The Diary Page is sitting on the couch.




Diary Page #3 - "Throwing Away Greatness"

In the same room as the previous Diary Page. From the couch, go left and in the corner will be some boxes and binders. The Diary Page is on one of the boxes. This will be just left of an Echo needed for Calling Out to Me trophy_gold.png.




Diary Page #4 - "A Well of Inspiration"

In the same room as the previous two pages. On the far side of the room, behind the table in the middle, is an IV Drip you will interact with for a Memory Shard. Just behind this will be some chairs underneath the window. The Diary Page is sitting on one of the chairs. 




Diary Page #5 - "A Method to Madness"

Shortly after picking up the previous Diary Page, you'll find yourself in a small room with a Globe in the middle. To the right of the Globe is the Diary Page. 




Diary Page #6 - "Growing Apart"

In the Dayroom. When you've gained access to the Teacher's Room on the right side as Material-World Marianne, the Diary Page will be on the desk on the far side of the room. This will be just left of a pair of glasses you need to interact with to fix a Memory Shard. 




Diary Page #7 - "A New Home"

After fixing the Music Sheet and restoring the Spirit-Well, you'll make your way through a swarm of moths and eventually end up in a large open room. Hug the right wall and in the back right corner will be the Diary Page sitting on a table.
