This is story related and cannot be missed. It is unlocked when you complete the main story mission, Captain, Go Home.
The story branches during the Naked Hunch story mission. You must side with one of two people. Both options result in obtaining the same achievements though the events differ substantially. One branch leads to The Beast That Shouted Love at the Heart of the World the other to For a Breath I Tarry. Completing one or the other of these mutually exclusive missions allows you to return to The Unreliable when you have completed all your other business on Eridanos. Once aboard, speak with ADA. Concluding the conversation completes the Captain, Go Home mission and unlocks the achievement.

This is bugged for many people. After killing the slug queen the game will crash at random times before you can get out the elevator. If you try to save the game after killing the queen, it will crash. I’m currently stuck and unable to complete the DLC, tried redownloading the game, playing on my Xbox One instead of Series X, no change. Googled it and see many complaining of same issue.

Thank you Allgorhythm. I deleted a bunch of old saves and then loaded up a save from earlier in the DLC and just ran through everything as you stated. Once I got out of the building after killing the queen it was definitely choppy and seemed to want to crash but I made it all the way back to my ship and popped the achievement. Thanks for the suggestion :)

Great. That’s good news. I’m sure a patch is coming soon but it’s frustrating to be at the end of a game & suffer technical issues that prevent you from completing it.

That's the second game I can't complete today due to glitches/crashes... Is it too much to ask for a game to work properly this days ?

Great. The more people that report the quicker we’ll get a patch. I’ve been doing some experimenting—playing on the X1X & the Series X. The problem occurs on both systems.

I trimmed my save file to make it as small as possible, keeping only my file at the start of the DLC (just in case), and the last point I've been able to save before the crashes start (just before mixing the cure)... It's still crashing like crazy. And I can't even rely on the auto-save, as there is none after dispersing the cure and I can't go far enough running to the factory, the game always goes bananas when I'm running on the 2nd bridge. So if there's another one entering the factory I can't reach it.
I'll get in touch with the dev too, hopefully it'll help speed things up as it did with Sunset Overdrive.

That’s unfortunate. The publisher, Private Division, & the Developer, Obsidian, are working on a fix. I would encourage you to submit a support request. You can monitor discussions on the issue in the Obsidian forums. I’ve posted the links in the 22 & 23 March comments above. You might mention specifically where the severe crashing started—the Pilothouse, the Distillery, or elsewhere. If you’re like others, you cannot manually save without crashing. If this is the case, mention it as well.

Got a email from Private Division announcing the release of a patch. I had no more game crashes with it installed and finished the dlc with out a problem.

Thanks. Your experience matches mine. The patch updates to version 1.5.2.733. I spent an hour playing through the two end-game branches. Neither one crashed in contrast to my earlier play-throughs that were plagued with incessant crashes.

I had numerous crashes myself. Without fighting, I rushed from the autosave just after the first elevator to the second elevator using the inhaler for the damage. After descending the second elevator, I rushed to the boss engaging no other enemies & killed the boss under tactical time dilation with a sniper rifle. I exited without engaging any minions. Fortunately, it did not crash and I got an autosave when I got back outside. I still had crashing problems getting back to the ship & had to use the same tactics--run to the ship, talk to no one, and get the auto-save upon entering the ship.
If you have an early save in the DLC, you might consider using it. You only need to investigate one suspect to name the murderer. That kicks off the endgame sequence. This could keep your save file smaller and prevent or reduce the crashes. Note that the bad ending is a little shorter & that may help too.

exorr but I know it is not a guaranteed approach. Unfortunately, the most reliable way to resolve this is to start a new game and speedrun through either the
Signal Point in Space or the
Radio Free Monarch main story mission. Make sure you have unlocked the Monarch Stellar Bay Landing Pad. At that point, you'll be able to start the DLC. The point is to do as little as possible and keep your active save file small to avoid the crashing problem.
Investigate only one suspect & report. You can then name the murderer and go into the Naked Hunch mission. Regardless of whom you side with, you'll fight the boss and unlock the Regicide achievement. After that, return to the ship and talk to ADA to get your final achievement.
I have contacted support and reported the issue providing a short clip to show how severe it is. If you want to submit a support request, here is where you do it:
