This achievement is earned during the main quest The Demolished Woman on Byzantium. To get this side quest you must attempt to turn in Phineas to the board through Udom Bedford. After getting his seal back from Gladys he’ll get you to go to Byzantium to meet Sophia Akande, who will then give you this main quest to prove yourself to her.
Sophia asks you to “deal with” a cartographer named Rachel Lockwood. If you head to her office and break in, you can find out that she is hiding in the maintenance tunnels under Byzantium. Head down there and “deal with her” however you see fit which will unlock the achievement. You can then return to Sophia when you’re finished to complete the quest.

You have until you start The City and the Stars mission to turn in Phineas and this mission gets locked out.

- To get this Side-Mission you must make this decision during the main quest:
[ Attempt to turn Phineas in to The Board through your conversation with Udom Bedford in the Halcyon Building on The Groundbreaker ]
- Doing this will get him to tell you that you must get his "Seal" from Gladys for just over 5000 bits I believe. Once you do that he will tell you to go to Sophia on Byzantium.
- She will then ask you to "Deal with the Cartographer" which caused Felix to get taken off of my squad (by choice lol)
- Following the quest you'll find she's hiding in the Byzantium Maintenance Tunnels... go down there (shown in the video) and take out that loose end to acquire this achievement :)

To get this achievement you must complete The Demolished Woman on Byzantium. To get there you must attempt to turn in Phineas to Udom Bedford when you first land on the Groundbreaker.
After attempting to turn in Phineas, Udom Bedford will ask you to to retrieve his seal from Gladys. This will cost 8000 bits. At this point in the game you probably don't have enough to pay this so Gladys will send you on the mission The Distress Signal as a way to get some.
The first step for The Distress Signal is to get ADA to play back a signal on your ship. Then will send you to Roseway. In Roseway, speak to everyone you can. You should then have 3 different objectives to obtain schematics for different people. When you find the schematics, you'll have to lie that you didn't find them so you can sell them to Gladys for bits.
After completing the mission, return to Gladys and sell the schematics to her. You will now have around 9000 bits and can buy back the seal. Udom Bedford will now send you to Byzantium to meet with Sophia Akande. She will give you the quest The Demolished Woman. All you have to do is murder (or spare) Rachel Lockwood, the cartographer.
Upon dealing with her the achievement will pop.

When you talk to Udom on the Groundbreaker. Atempt to turn in Welles.
He'll direct you to Byzantium to talk to Sophia.
Agree to help and the quest "The Demolished woman" starts.
Go to her office abd steal her research first off.
The receptionist sends you over to the bar across the road for her location. If you fail the persuasion check, you can pick pocket the bartender and make your way to the sewers.
On a new playthrough I discovered that you can skip the above and pick pocket the receptionist and let yourself in to her office upstairs (using the lift). Then her location is revealed on her terminal. Next to the Terminal is the research Sophia wants, which will be most helpful.
Find Rachel and kill her for the trophy and Sophias approval.
ExtremePhobia pointed out that this trophy also unlocks if you let the good Doctor live and just turn in the research.

The achievement unlocks during The Demolished Woman main story quest (and before you complete it) when you meet Rachel Lockwood and decide her fate. Your decision does not matter. The only criterion is that you meet with Rachel and decide what to do with her. It is missable because you can reach the point of no return in the game without completing the quest.
In order to get The Demolished Woman quest, you must make the decision on Groundbreaker to tell Udom Bedford you have info regarding Phineas’s whereabouts. The game’s story line branches at this decision point. If you elect not to divulge info on Phineas’s whereabouts to Udom, you do not acquire The Demolished Woman quest and miss the achievement.
Once you tell Odum you know where Phineas is, he will give you the Balance Due quest. You must retrieve his Board Seal. When you give it to him, he gives you the Navkey to the Adjutant Akande Landing Pad in Byzantium. The short Puppet Masters main quest automatically begins. For the upcoming events, it is of value to take Felix with you for his lockpicking bonus and to improve his opinion of you. If your lockpicking is below 60 or so, take Parvati as well—she also boosts that skill.
You land on Byzantium and meet with Sophia Akande. This completes Puppet Masters and automatically initiates the The Demolished Woman main story quest.
Sophia wants you to retrieve some documents for her. Go up the elevator in the Bureau of Exploration to Rachel Lockwood’s office. You can pick the lock (skill check of about 60) and obtain the documents from her desk. At this point, you can return them to Sophia to complete the quest although she will not be entirely pleased. Nevertheless, you will have declined to ‘deal’ with the cartographer and, thus, will not unlock the achievement. Use the unsecure terminal on her desk to learn Rachel went down into the [maintenance] tunnels.
You are not required to talk to the ‘receptionist’ on the ground floor. But, if you do, with high intelligence, you can deduce he’s a contract killer hired by Sophia. You can also pick the lock of a nearby room to find the body of the real receptionist and figure out the man behind the window, Sherman Bradshaw, is an assassin.
Bradshaw will give you the keycard to Rachel’s office if you ask him and pass a persuade check of about 40. This should be easy with Felix and Parvati in your party—they both provide bonuses to the Persuade skill. Or, you can enter the receptionist’s office, sneak up behind Bradshaw, and pickpocket him to get the keycard by stealth.
More importantly, once you have snuck up behind Bradshaw, kill him. This is not required but simplifies the quest in general and unlocking the achievement in particular. You can attack him head on, if you want, but he’s a slightly pesky opponent and the sneak kill is more efficient.
Having learned of her location in the Maintenance Tunnels, follow the quest marker to Rachel. If you kill her before talking to her, the achievement will pop immedately. If you talk to her, you can decide to spare or kill her. All of these choices will unlock the achievement at the time you commit to a decision.
If you didn’t kill Bradshaw, he will show up. If you allow him to kill Rachel, you will still get the achievement. It’s not entirely clear why you need to crack the safe at all - there is a lot of money in the game, you can simply buy a seal for 8000.
