MISSABLE During the Wormhole level you will come to a point where you find a musical instrument and walk up a spiral staircase. Towards the top, there will now be a series of holes and you need to enter in the correct order. Each correct choice will raise you up one level, while choosing the wrong one will cause you to start over at the bottom.
The order is 1, 1, 2, 3, 5 (counting from the left). Choose this path on your first try and the achievement is yours. There is a clue on a piece of paper on the ground, near the first group of holes. These are the first few numbers in the Fibonacci Sequence, which is closely tied to the Golden Ratio (1.618).
Lastly, if you are stuck at any point in the game, you can turn on Guided game mode in the game options. Then press LB to see where to go next. This does not void achievements.

You will get this in Chapter 5, Wormhole (after BATTERY CHARGER
). You'll eventually come to a very red area where you need to climb up a spiraling... let's just say walkway. After you pick up the clarinet, go into the suction hole and climb up the rest of the dark red pathway until you reach the flute with a piece of paper on the ground. Interact with the paper to find the solution to the puzzle for this trophy:
Make a note of the dots on the bottom and you'll get a pattern of 1 - 1 - 2 - 3 - 5.
Pick up the flute and when you play it, 5 holes will appear on the back wall. Number these 1-5, going left to right.
When you interact with a hole, you'll "go through it" and come out one level higher with another set of 5 holes (these will all be numbered the same way). You need to go through them in the order shown on the music paper you looked at earlier (1-1-2-3-5). Do this in the right order on your first try and the trophy will unlock.
