You need to punch seven (I think) people. The first seven you can hit are:
- The Lieutenant General and the Field Marshal right at the start
- I don't think the three grovellers at the start count, but you might as well slap them up. Doing so will get you the Spank the Bishop achievement
- The cripple with the oars outside the field hospital
- Man tied to tree (with apple on his head)
- The gatekeeper with the massive key
- The Ferryman (by the Basilica not the one at the start)
- One of the scholars inside the Basilica
- The two cardinals inside the Basilica

And onnovdw notes that also killing everyone will get you the Serial Killer achievement too.

It work if you punch seven time the first pnj, so if you don't have this one you can do it on the early game instead of slapping every pnj.

This achievement is actually very simple. You do NOT need to hit 7 different people as hitting one 7 times will work just as well. I personally did it when I first got to the Basilica by hitting the scholars 7 times. All you need to do is click the scholars to the left of the cardinals guarding the door and select the hand icon 7 times to slap them and repeatedly insult them and the achievement will unlock.
