This Achievement sounds daunting, but it's actually very easy to get. I would grab this one alongside Hackett's Quarry Massacre and The Final Girl on the same Playthrough (go into "Chapter Select" after the final scene with Eliza/The Hag of Hackett's Quarry!).
Prologue:
No set-up required. Be rude to Travis (The Cop) if you would like to, but this is a breezy Chapter to get through.
Chapter 1:
Steal the rotor-arm from the van as Jacob. This is so we can kill him off very early in Chapter 3! Break into the cabin as Abigail. Grab the stuffed toy from inside the cabin. This is absolutely crucial, so don't forget it.
Chapter 2:
No set-up required.
Chapter 3:
Jump back into the water to retrieve the rotor-arm as Jacob. Make sure you grab it when the QTE pops up so that his hand becomes entangled in the barbed wire. Watch as the timer ticks away and Jacob dies. Congratulations! You've killed your first counselor!
Chapter 4:
