They changed how these work from the previous games. Now, you simply hold A until you see the red aura of the person/ creature pursuing you disappear. Most often this comes during the final 25% of the timing bar of the event.

FYI, having Don't Breathe events set to Auto in the Accessibility settings prevents Don't Breathe events from counting towards this achievement.
