Story related, cannot be missed. I have put the necessary steps below in the spoiler tags for anyone having difficulty with the puzzle.

When you enter the guest rooms area for the first time, you will be tasked with preventing the third murder, that of Willow Blue. Willow commits suicide in the bell room (the bottom right corner of the map of the first floor) at approximately 7pm, so you have until then to prevent her death.

Firstly, ensure you have synchronised the pocket watch with the clock in the guest rooms: this can be found in the long room with two fish tanks at the most southern part of the first floor map. The Winding Key for this clock can be found in the room with the locked chest and the large fish tank on the northern wall, which you enter from the northernmost door on the left side of the eastern corridor in this area. Then follow the steps below:

- You must observe Willow entering or leaving a hidden door in the painting room. You can either hide in a wardrobe in the rightmost corner of this room, or look through the room's right (east) door (by pressing

) without entering. Willow enters this room shortly after 2pm, and exits the secret passage shortly after 4pm: observing her at either of these times will unlock the knowledge of the secret passage.
- After Willow's death at 7pm, enter the bell room and collect (using

) the Bone Key she leaves behind near the lever where the bell rope was attached.
- Enter Willow's guest room (from the bell room, exit via the north door, enter the first door on the left of the corridor you enter, then walk through the painting room and use the door on the left) and use the Bone Key (select it as your active item by pressing

until it appears in the bottom left corner of the screen, then press

) when standing by the desk. You will collect the Hungry Charm recipe, and two 'meal locations' will appear on your map.
- Restart the day (

)
- From the clock in the guest room, immediately take the left door. You'll find the Hungry Charm on the floor in the centre of this room.
- Return to the room with the clock, and, with the Hungry Charm as your active item, interact with the right fish tank to give the Charm its first meal.
- Take the door on the right of this room, take the north door out of the bell room, then enter the second door on the left of the east corridor (the room where you found the Winding Key). Interact with the fish tank on the northern wall, and the item will now be named the 'Sated Hungry Charm'.
- Exit this room to the eastern corridor, then take the lower door on the left to return to the painting room. Enter the hidden door here, and you'll see a large skull on the wall blocking a door. Use the Sated Hungry Charm to cause it to reveal the door, then enter the room.
- Go up the stairs to the right of the room, and wait for Willow to enter the room. Interact with the valve on the left of the walkway you're on. This redirects the pipe to the left, above the candle. Continue to hold it here as Willow seemingly talks to herself until the water flows again, extinguishing the candle. A cut scene will play, after which you'll receive the achievement.

This does not unlock for me despite me saving Willow, gaining her powers and having the game move on to saving Tequila. Any suggestions?

That's really strange; I didn't have any issues with achievements unlocking at the right time for this game. Given it's a story/progression-related achievement, it might be worth trying a few of the standard tricks (restarting, power cycling, etc.) to force your Xbox to sync with the cloud, in case it's just gotten stuck connecting to the server.

Story-related and unmissable. You’ll earn this trophy after preventing the third murder, that of Willow Blue.

To prevent this murder, you’ll need to take the following steps (please be aware, these provide an explicit solution to the scenario):

Observe Willow as she enters the painting room directly east of her guest room, which will be at around 2.20pm. You’ll see her entering a hidden door in this room, allowing you to use this door yourself.
Once Willow has committed suicide, collect the bone key that she leaves behind.
Go to Willow’s room and use the key to open her desk, collecting the hungry charm recipe. You’ll now see two “meal” locations marked on your map.
Reset time.
Go one room west from the clock in this area, collecting the hungry charm that’s been left on the floor of this room.
Take the charm to the two marked “meal” locations, interacting with the fish tank to “feed” the charm.
Enter the hidden door in the painting room, then use the now-sated charm on the skull to unlock the entrance here.
Go up the stairs and interact with the valve, holding on to it in its new position until Willow enters the room and attempts a ritual. The redirected water flow will extinguish the candle, stopping the ritual.
