Once you hit level 5+ Gardening, Simply grow two plants (buy seeds off the internet and plant them into pots/plots) then select one and click Take Cutting, then go to the other and select Graft. Then voila, achievement unlocked, the new plant doesn't even need to fully grow.

Grafting doesn't unlock until Gardening level 5.

To get this trophy, you’ll need the “Take Cutting” and “Grafting” actions. Both are available once you reach Level 5 Gardening Skill. Click on any fully grown Flower/Vegetable Plant and select “Take Cutting”. Once you have it, click on a different full grown Flower/Vegetable Plant (of a different species) and select “Graft”. You CAN graft Flowers with Vegetables.
