During the seventh story mission - Self-Defense - After Milton Pierce asks you to kill his mother, go to her mansion and serve her poisoned chianti. At no point can you tell her what her son is planning, or you'll lose out on this trophy. Also, you mustn't hesitate. If you get cold feet and back out of the conversation before you give her the poison, you won't get another chance.

During the story mission "Self-Defense", Pierce Milton will ask you to get rid of his mother. Visit her house, agree to bring a bottle of wine from the basement and poison her by adding poison to it.
