The Sinking City

Pass by the first two tears you see when you come down the hill and exit the far side of the area where you will see a third tear.

Make sure you save before you go in and you will be able to reload and get the other two endings achievements without having to replay the whole game.

Taking this tear ends the game by leaving the city
