For this one, you need to kill all enemies in the first area. Kill EVERYTHING you see along the way.
The achievement will not pop until after you finish the first area. Annoyingly, it didn't pop even when the area 1 achievement popped, but after I killed a few more enemies afterwards. If it doesn't pop for you when you get that achievement, just keep going.
Unfortunately, with no chapter select, if it doesn't pop you're going to have to restart the game.

Did the same thing for me and I definitely killed every enemy. Roughly 5-6 more in the next area and it popped though, so I can second your experience.

I wonder if they patched this because for me the achievement popped as soon as I killed the last enemy on the first level.
