When you get to Flashpoint Cave 1, you will come to a series of 4 doors. Each one is operated by a simple ground switch. After the 4th door, a smoke monster will begin chasing you. Luckily, it's slow. Unfortunately, it will kill you. Don't try hiding in a side tunnel.
Run back through all the rooms, up the stairs, and to the room with the pillars. Stand on one side of the pillars, enticing it to follow you on one side of the room. Then run around to the other side of the room, down the stairs, and all the way through.
You will come to another room where another smoke monster is lurking. This is a much larger room. Don't worry about exploring in here, just focus on following the signal. Eventually you'll reach a pool you need to swim across. When you get to the other side, and open the mechanism, the achievement will pop.
Then you can save on the other side of the door.
Run back through all the rooms, up the stairs, and to the room with the pillars. Stand on one side of the pillars, enticing it to follow you on one side of the room. Then run around to the other side of the room, down the stairs, and all the way through.
