As you go through the game, you'll see a bunch of QR codes on the walls. Look at every one you see. I don't know how many you need, but you definitely don't need all of them. I got this about 80% of my way through the hubs, before I went to the tower. YMMV.

Keep looking at as many of the QR codes around every level as you can and you'll eventually get this. The amount needed is somewhere around 75, so it shouldn't take that long if you're not running past them all.
All over the various areas of the game (the main areas, the puzzle hubs, and even in some puzzles themselves) you will undoubtedly notice QR codes on the wall that you can point at to scan and read them. For this achievement, you just need to scan and read many of them. I know that is vague, but you thankfully don't need to scan anywhere near all of them. I unlocked this achievement near the start of World C, and there are many QR codes in World C and a lot more on the various floors of The Tower. Make it a point to scan all of the QR codes you happen across and you'll unlock this achievement before finishing the game.
I don't know how many you need or how many there are, but you should get this sometime during your first Ascension playthrough. I ran by most if not every code. When I went back to do a clean up of the levels, I got the trophy pretty early on as I started reading every code I came across. You really do not need many, I want to say less than 75 since I ran by a ton. Reports say World C is the expected place to get, but there are a lot of hidden QR codes in the levels, not to mention codes hidden in the hub world, messenger islands, star worlds etc.
