It seems that all 3 kills related achievement are buggy and just can refuse to pop randomly.
So you may have 100/200 kills achievement but have not the one for 50 kills.
I believe the solution is only to start another playthrough in such case...

I doubt there is any way to miss this achievement. There is way more than 200 humans in the game and you are forced to fight almost all of them. Plus they respawn if you come back to an earlier area.
